Living Will-Living Will Forms

I understand what a living will is about:prolonging life or not,feedings,power of attorney,agent’s obligations and when it takes effect but where it says agent’s authority it says “My agent is authorized to make all health care decisions for me,including decisions to provide,withhold or withdrawal artificial nutrition and hydration and all other forms of health care to keep me alive except as I state here:”Withhold all treatments even at a terminally ill stage including cancer,infections except the poa to hospice care with no feeding tube.If at any time I may be close to death I wish to resolve issues with family members.I wish to be cremmated with a Christian Funeral and pass away at home with hospice care.”

From what I wrote above is that to still be followed when I state withhold all treatments even without a refusal of medical treatment form or do I still need one?If so,what other things can I write on the living will to make things clearer as well as a Refusal of Medical Treatment Form?
I don’t have money for a lawyer and I don’t even work.What’s another way around it?I can’t ask my parents to do it because I know they won’t consider it.
